When a generation fails, isolate whether the cause is input quality, product state, access, or an external provider dependency.
Triage order
- Re-check the active account and feature access.
- Re-check the prompt or reference asset quality.
- Retry once with minimal changes.
- Move to a narrower workflow if the larger flow is unstable.

- Approximate time of failure
- Whether the issue reproduces
